UHID stands for Unique Health Identification. The hospital issues a unique UHID number to each patient, which helps in tracking the history of patient's treatment at the hospital.

The purpose of the e-Upchaar online registration portal is to provide patients with a facility to book online appointments and accessibility to view diagnostic reports. The main vision behind the development of this extensive portal is to free patients from the hassle of standing in long queues for registration.

To view your appointments, you need to click on View Appointment icon provided on the home page. The system will prompt you to enter UHID or Phone number.  After entering UHID, an OTP is send to your registered mobile number. Once you have entered the OTP, the system will direct you to the View Appointment page, where you can view the list of appointments.

You can access the diagnostic reports (Laboratory and Radiology reports) by clicking on the Diagnostic Reports icon, provided on the home page. The system will prompt you to enter UHID or Phone number.  After entering UHID, an OTP is send to your registered mobile number. Once you have entered the OTP, the system will direct you to the Reports section, where you can view and access the Laboratory and Radiology reports.

A patient can simply book an appointment by registering on the e-Upchaar Online Registration portal. Both the existing user (with UHID) as well as new user (without UHID) can book an appointment through the web portal’s user-friendly interface.

A patient can get a UHID at the time of registration. The patient can book an appointment through the e-Upchaar online registration portal. When the patient visits the facility, he/she will receive the UHID from the registration counter.

For new patients, the Mobile number is mandatory for booking an appointment. An OTP will be sent to the patient’s registered mobile number.  After providing the personal details, the user will be able to book an appointment.

For existing patients, 12-digit UHID number generated through the e-Upchaar application will be required. After providing the UHID and on successfull verification, a user can book an appointment.
